United Healthcare will continue as the vendor for the LSU Life Insurance for the 2022 calendar year.  No one wants to think about the possibility of death, but it's important that you ensure your family's financial security in the event something happens. Term life insurance offers financial security for you and your family. Term life insurance is offered to eligible employees through two different vendors, UnitedHealthcare and Prudential.

UnitedHealthCare (UHC)
LSU partners with UnitedHealthcare to provide you and your family with Term Life coverage at affordable rates. This coverage provides affordable financial security for your loved ones, especially when your family depends on your income. This coverage is offered to eligible employees, spouses and child(ren) Employees and dependents cannot be double covered. This benefit is 100% employee paid.

Employee coverage
This plan provides guaranteed issue of coverage up to the lessor of 3x your annual salay of $350,000, if you enroll within 30 days of your benefits eligibility. If you do not enroll as a new hire, you can enroll anytime with evidence of insurability. Premiums are age-based and will adjust according to age. Spouse coverage
Spouse coverage is available on a guaranteed issue basis for up to 50% of the amount of employee coverage with a maximum of $100,000, if enrolled within the first 30 days of benefits eligibility or within 30 days from your date of marriage. If you do not enroll them within 30 days of their benefits eligibility, they can apply for coverage anytime with evidence of insurability. Premiusm are age-based and will adjust according to age. Beneficiary designation for spouse coverage is automatically listed as the employee.

Children coverage
Employees may elect child(ren) life insurance coverage in the amount of $5,000, $10,000, $15,000 or $20,000 for each eligible dependent child - no medical information required. Dependent children are covered from 14 days to 26 years of age. Premiums are based on the amount of coverage and will not adjust

Beneficiary designation for child(ren) coverage is automatically listed as the employee.

Employee Eligibility

Any active employee of LSUS is eligible for dental insurance provided the following:
Employed at 75% of full-time effort or greater (at least 30 hours per week);
Appointed for a duration of at least one semester or 121 days or greater Enrollment

Dependent Eligibility

- The covered employee's legal spouse
- A child from the date of birth up to 26 years of age (regardless of student status or tax status)

New Hires and Mid-Year Changes

New Hires
must complete enrollment in Workday within the first 30 days of employment. After the 30 days of employment, employees will need to apply for coverage through Medical Underwriting.
Mid-year changes are allowed for IRS "qualifying events" within 30 days of the event. Qualifying events are: change in family status (birth, marriage, divorce, legal separation, change in employment status of employee or spouse), changes required by judgment, FMLA qualified leaves of absence, significant cost or coverage changes. Proof of the qualifying event is required in order to make a mid-year change.

Effective Dates of Coverage
Annual enrollment changes are effective January 1st of each calendar year. The effective date for new hires is the first of the month following a full month of employment. For example, a person who is hired on August 20th, and elects coverage within the first 30 days of employment will have coverage begining October 1st.

Terminating employees:  Portability / Conversion Option
Coverage terminates on the last day of the month in which the person was employed at LSUS.  Terminating employees must be under age 70 and in good health to have the portability option (continue term life insurance). Another option is conversion (converting to a whole life policy).

Terminated employees who wish to continue life insurance via portability or converstion must send the completed applicable form to UHC within 31 days of coverage termination. 

Retirement
Employees under age 70 at the time of retirement (and in good health) can elect to port this policy to an individual life insurance policy. The retiree must submit the portability form must be to UHC within 31 days of coverage termination to continue coverage. Payments are made directly to United Healthcare. 

Employees age 70 or over at the time of retirement can convert the policy to a whole life insurance policy. The retiree will need to submit the conversion form to request a quote for whole life insurance coverage. This must be done within 31 days of coverage termination. Payments are made directly to United Healthcare.

Prudential Life Insurance Plan Information
Two different plans to choose from: the Basic Plan & the Basic + Supplemental Plan
Basic Plan: $5000 of coverage
Basic + Supplemental Plan: Coverage is 1.5 times your salary. The maximum amount of coverage is 1.5 times salary up to $50,000 Can continue into retirement at same rate. Employee/family member needs to contact the Human Resources to file a claim.

- Dependent verification is required for the Office of Group Benefits dependent life insurance. Employees must present an original birth certificate or marriage license in order to obtain coverage.

New Hire/Newly Eligible Employee

Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligible for benefits.

Existing employee/family member
If an employee did not enroll in coverage within 30 days of their eligibility to obtain benefits, they will have to undergo review of their medical history for coverage.

Terminating Employment
If the employee loses coverage or leaves LSUS employment the employee/family member will be able to port their coverage for Prudential. Prudential's rates will change because coverage is being ported. Employee must apply for Portability within 31 days of the coverage termination date. Employees also have the option to convert the group term life policy to an individual plan.

Retirees
This life insurance is eligible to be continued into retirement. It can be deducted from LASERS and TRSL paychecks. ORP retirees will have their premiums drafted from the bank account each month through LSUS payroll and will need to complete an AS-35 form. LSUS HR will send to LSU insurance.
