Important Application Steps
Pay the $20 application fee ($30 for international students).
Submit official transcripts from each college or university attended. Request an official transcript from all colleges or universities previously attended whether or not credit was earned. Transcripts can be mailed directly to the LSUS Admissions and Records Office or sent electronically to admissions@lsus.edu via secure means, i.e., eSCRIP-SAFE, Parchment, etc. If you attended institutions outside of the United States, submit a completed course-by-course evaluation of all credits and/or degrees earned. Please refer to the International Graduate Checklist for instructions on ordering an official course-by-course evaluation, as well as the list of approved evaluation companies.
Submit Immunization Records form. Provide proof of immunizations, or sign the immunization waiver through your myLSUS account by navigating to the “Records” section, selecting "Forms," and choosing the "Immunization Form."
