LSUS works to make your student experience positive and meaningful. However, there may be times when you want to share a concern about a student service, a co-curricular activity, academic experience, or campus incident. The policy (P.S. 4.17.00) governing student complaints is outlined in the University Policy Statements. LSUS distinguishes between student complaints and student appeals of University policies and decisions. A student appeal, whether for an exception to a policy, a decision made by a judicial body, a grade, a financial aid award, or a refund policy, is not considered a student complaint. There are specific procedures in place for students who wish to make an appeal; these procedures vary according to the type of appeal. Information about appeals procedures are located in the relevant sections of the Student Handbook and/or in the LSUS Catalog. In all instances, the Dean of Students is available as a resource to students and assist them in navigating complaints or appeals in a prompt and effective manner. If the complaint falls under the jurisdiction of a more appropriate procedure, such as Title IX or an appeal process, the student will be referred to the appropriate campus process.

Students wishing to file a formal, written complaint may do so using the form below. All written complaints submitted on this official form will be reviewed. Once reviewed, the formal complaint will be directed to the responsible director, chair, or administrator (according to the table below) who will then investigate and resolve the complaint. Complaints submitted by any other means are defined as informal grievances and do not require proper adjudication by the University.
