Life Insurance and AD&D (United Healthcare and Prudential)

United Healthcare will continue as the vendor for the LSU Life Insurance for the 2018 calendar year.  

Important Annual Enrollment Information
:
- Employees who have children covered on their life insurance must re-elect the coverage in Workday and select the covered dependents from the drop-down box or the coverage will be waived for the 2018 plan year. Click here for a Workday Open Enrollment job aid. 
- Employees who have coverage can increase their existing coverage during the month of October 2017.  
- Employees who retire on or after January 1, 2015 cannot continue this life insurance into retirement! 
- All changes must be submitted in Workday no later than close of business on October 31st.

If you are not enrolled and would like to enroll:
Employee:

Employee must apply with medical underwriting (complete an evidence of insurability form). 
Spouse:
Spouse must apply with medical underwriting (complete an evidence of insurability form). 
Eligible Children (up to age 26):
You can elect $5,000 or $10,000 or $15,000 or $20,000 upon employee approval. No medical underwriting is required.

If you are currently enrolled and would like to increase coverage:
Employee:
You can increase your coverage in Workday up to an additional $10,000 without medical underwriting as long as you do not exceed 5X your salary or $500,000. 
Spouse:
You can increase coverage in Workday by $5,000 as long as your total amount of coverage doesn't exceed $100,000. Spouse's coverage can't exceed 1/2 of employee coverage.
Eligible Children (up to age 26):
You can elect $5,000 or $10,000 or $15,000 or $20,000. No medical underwriting is required.

Premiums

This is an age-banded product - meaning that premiums increase as you get older. click here to view a detailed premium calculation sheet.

Employee Eligibility
Any active employee of LSUS is eligible for dental insurance provided the following:
Employed at 75% of full-time effort or greater (at least 30 hours per week);
Appointed for a duration of at least one semester or 121 days or greater Enrollment

Dependent Eligibility

- The covered employee's legal spouse
- A child from the date of birth up to 26 years of age (regardless of student status or tax status)

New Hires and Mid-Year Changes

- New Hires must complete enrollment in Workday within the first 30 days of employment. After the 30 days of employment, employees will need to apply for coverage through Medical Underwriting.
- Mid-year changes are allowed for IRS "qualifying events" within 30 days of the event. Qualifying events are: change in family status (birth, marriage, divorce, legal separation, change in employment status of employee or spouse), changes required by judgment, FMLA qualified leaves of absence, significant cost or coverage changes. Proof of the qualifying event is required in order to make a mid-year change.

Effective Dates of Coverage
Annual enrollment changes are effective January 1st of each calendar year. The effective date for new hires is the first of the month following a full month of employment. For example, a person who is hired on August 20th, and elects coverage within the first 30 days of employment will have coverage begining October 1st.

Terminating employees:  Portability / Conversion Option
Coverage terminates on the last day of the month in which the person was employed at LSUS.  Employees who are terminating employment (not retiring) have the option to continue this insurance through Portability.  Terminating employees must be under age 70 and in good health to have the portability option.  The portability must be completed within 31 days of the termination of coverage. Retirees
Employees who retire on or after January 1, 2015 cannot continue this life insurance into retirement through portability but have the conversion option! Retirees who currently have the Hartford Life Insurance will be able to continue the same coverage through United and will pay premiums directly to United Healthcare.

ACCIDENTAL DEATH AND DISMEMBERMENT (Stand Alone Plan)

This benefit pays in the event of death or dismemberment resulting from an accident.

Important Annual Enrollment Information:

No action is required if plan members would like to continue their current AD&D coverage.
- Employees who do not have coverage can enroll during annual enrollment or anytime throughout the year.
- Employees who have coverage can increase or decrease their existing coverage during annual enrollment.
- There is a premium reduction this year!
- All changes must be submitted in Workday no later than close of business 10/31/2017.

New Hires and Mid-Year Changes
New Hire:
May enroll at any time without medical underwriting. Benefits are effective the first of the month following date of hire.
Current employee: May enroll at any time without medical underwriting. Benefits are effective the first of the month following when enrollment form is signed.
Newly eligible employee or family member: May enroll at any time without medical underwriting. Benefits are effective the first of the month following when enrollment form is signed.

Terminating Employment
If you leave LSU employment, your AD&D policy will end at the last day of the month in which you leave LSU. You will be able to convert your AD&D policy after employment. Please complete the form below and forward it to United directly to convert your policy.

Retirees
Retirees are able to continue AD&D coverage into retirement. AD&D premiums can be deducted from TRSL or LASERS paychecks.  ORP retirees will have these premiums drafted from the bank account each month through LSUS payroll.  Retirees must elect to continue coverage.

Prudential Life Insurance Plan Information

Two different plans to choose from: the Basic Plan & the Basic + Supplemental Plan
Basic Plan: $5000 of coverage
Basic + Supplemental Plan: Coverage is 1.5 times your salary. The maximum amount of coverage is 1.5 times salary up to $50,000 Can continue into retirement at same rate. Employee/family member needs to contact the Human Resources to file a claim.

Employed at 75% of full-time effort or greater (at least 30 hours per week);
Appointed for a duration of at least one semester or 121 days or greater

Dependent Eligibility / Dependent Verification
- The covered employee's legal spouse
- A child from the date of birth up to 26 years of age (regardless of student status or tax status)
- Dependent verification is required for the Office of Group Benefits dependent life insurance. Employees must present an original birth certificate or marriage license in order to obtain coverage.

New Hires and Mid-Year Changes

New Hire/Newly Eligible Employee

Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligible for benefits.

Existing employee/family member

If an employee did not enroll in coverage within 30 days of their eligibility to obtain benefits, they will have to undergo review of their medical history for coverage.

Terminating Employment
If the employee loses coverage or leaves LSUS employment the employee/family member will be able to port their coverage for Prudential. Prudential's rates will change because coverage is being ported. Employee must apply for Portability within 31 days of the coverage termination date. Employees also have the option to convert the group term life policy to an individual plan.

Retirees
This life insurance is eligible to be continued into retirement. It can be deducted from LASERS and TRSL paychecks. ORP retirees will have their premiums drafted from the bank account each month through LSUS payroll. Life insurance claims are processed through the LSUS HR office.
