Long-Term Care and Long-Term Disability

Long-Term Care (UNUM) and Long-Term Disability (The Hartford) Insurance 

Long-Term Care Form (UNUM)
Long-Term Care (LTC) insurance is important for people of all ages if they want to preserve financial security and independence in the event of an extended disability.  And despite popular misconceptions, the need for long-term care insurance isn't just for the elderly as many working age adults have disabling illnesses and injuries as well.  This benefit is designed to help you pay for care services, such as in home care or nursing home care. 

Eligibility
Any active LSUS employee is eligible for Long-Term Care coverage provided the following:
- Employed at 75% of full-time effort or greater (at least 30 hours per week)
- Appointed for a duration of at least one semester or greater (121 days or greater)

Employees, spouses, parents and grandparents may enroll in this plan.  Employees and spouses are guaranteed issue within the first 30 days of hire.  Coverage for parents and grandparents is contingent upon completion of an evidence of insurability form and being approved by underwriting.  Coverage for employees and spouses after the first 30 days of employment is contingent upon completion of an evidence of insurability form and being approved by underwriting.

Long-Term Disability  (The Hartford)
The Hartford administers the Long-Term Disability Plan. LTD provides 60% of your salary if you are unable to work due to a medical condition.  The benefit is payable once 90 days has passed and your sick leave is exhausted.  New hires are issued guaranteed enrollment within the first 30 days of employment.

Long Term Disability Personal Health Application
If you are interested in applying for Long Term Disability after the first 30 days of employment, you will need to complete the Personal Health Application and the Long Term Disability Enrollment form to apply for this insurance.  LTD provides 60% of your salary if you are unable to work due to a medical condition, a 90-day waiting period has passed and your sick leave balance is exhausted.

Long Term Disability Conversion Form   (Terminations)
If the employee leaves LSU employment or loses coverage, the employee will be able to convert their long-term disability through The Hartford.  There will be a change to benefits and premiums when the policy is converted. The employee has 30 days from when they lose coverage to convert their LTD policy
