Office of Group Benefits: Term Life Insurance (Prudential)


Prudential Life Insurance Plan Information

Two different plans to choose from: the Basic Plan & the Basic + Supplemental Plan
Basic Plan:  $5000 of coverage
Basic + Supplemental Plan: Coverage is 1.5 times your salary. The maximum amount of coverage is 1.5 times salary up to $50,000 Can continue into retirement at same rate.  Employee/family member needs to contact the Human Resources to file a claim.

Eligibility
Any active employee of LSUS is eligible for life insurance provided the following:
Employed at 75% of full-time effort or greater (at least 30 hours per week);
Appointed for a duration of at least one semester or 121 days or greater

New Hire/Newly Eligible Employee
Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligible for benefits.

Existing employee/family member
If an employee did not enroll in coverage within 30 days of their eligibility to obtain benefits, they will have to undergo review of their medical history for coverage.

Terminating Employment
If the employee loses coverage or leaves LSUS employment the employee/family member will be able to port their coverage for Prudential. Prudential's rates will change because coverage is being ported. The employees has 30 days from when they lose coverage to port their coverage.

                                                        LSU System Voluntary: Term Life Insurance (The Hartford)
                                                         
The Hartford is the plan administrator for the LSU System Life Insurance Plan. All Hartford Life insurance forms and additional information are located on the www.imala.com/lsu website. Please visit the Life Insurance Benefits section.

Annual Enrollment - Important Announcement!!
EMPLOYEES CAN "GET IN FOR TEN!" That means that employees may elect $10,000 WITH NO HEALTH QUESTIONS ASKED UNTIL OCTOBER 31, 2013. This is a very rare opportunity!

Employees who are currently enrolled in the plan may increase coverage up to 1x their salary without underwriting as long as they don't exceed 5x their salary up to $500,000. All enrollment forms must be submitted to the Office of Human Resource Management no later than 4:30 pm on October 31, 2013.

Plan Information
Elect up to 5 X annual salary up to $500,000 if you enroll within 30 days of date of hire.  Premiums are age-based and will increase according to age. Can elect coverage amounts for spouse and children. Note : It is the employees responsibility to notify the Payroll Department to stop deductions when the youngest child is no longer eligible. Employee/family member needs to contact the Human Resources to file a claim. 

New Hire/Newly Eligible Employee

This information is also available in the Office of Human Resource Management, Administration Building, AD 109.

Terminating Employment

                                       Accidental Death & Dismemberment (AD&D) - The Hartford

This benefit pays in the event of death or dismemberment resulting from an accident.

Eligibility
Any active employee of LSUS is eligible for life insurance provided the following:
Employed at 75% of full-time effort or greater (at least 30 hours per week);
Appointed for a duration of at least one semester or 121 days or greater

Enrollment
New Hire: May enroll at any time without medical underwriting. Benefits are effective the first of the month following date of hire.
Current employee: May enroll at any time without medical underwriting. Benefits are effective the first of the month following when enrollment form is signed.
Newly eligible employee or family member: May enroll at any time without medical underwriting. Benefits are effective the first of the month following when enrollment form is signed.

Terminating Employment
If you leave LSU employment, your AD&D policy will end at the last day of the month in which you leave LSU. You will be able to convert your AD&D policy after employment. Please complete the form below and forward it to Hartford directly to convert your policy.

Retirees:
You will be able to continue your AD&D coverage for you and your eligible family members. Retirees must elect to continue coverage.
