2014 Annual Enrollment be held from October 1, 2013 - October 31, 2013 with changes being effective January 1, 2014. 
If you are already enrolled in dental and you wish to continue your coverage, you do not need to do anything. There is an 8% premium increase for the 2014 calendar yearIf you wish to enroll, change dental plans or cancel coverage, please see below for the Enrollment / Change form. All changes must be submitted to the Office of Human Resource Management no later than 4:30 p.m. on October 31, 2013.

New Enrollment or Making Changes
Please use the attached form to enroll or make changes to your current dental plan. If you are already enrolled in the dental plan, your coverage will roll over to the next plan year.

                                              DENTAL PLANS (BASIC PLAN OR ENHANCED PLAN)

Dearborn Dental (Basic Plan)
The basic plan offers a lower premium option to cover the basic care (2 cleanings per year and x-rays). This plan has a one-time $100 deductible for services other than cleanings. This may be an economical option for employees who do not need to have any dental work done.   Plan members may see their dentist of choice.  Tip: Minimize your out of pocket costs by using in-network providers!

Dearborn Dental (Enhanced Plan)
The enhanced plan is a slightly higher premium option that still offers the basic care with better coverage for additional services such as orthodontia, fillings, crowns, etc. There is no deductible on this plan. There are no premium increases this year!  Plan members may see their dentist of choice. Tip: Minimize your out of pocket costs by using in-network providers!

Eligibility
Any active employee of LSUS is eligible for dental insurance provided the following:
Employed at 75% of full-time effort or greater (at least 30 hours per week);
Appointed for a duration of at least one semester or 121 days or greater Enrollment

New Hire/Newly Eligible Employee: Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligible for benefits.

Existing employee/family member:  Employees can only enroll during annual enrollment which is within the month of October. Benefits will be effective January 1st of that year. If the employee/family member lost coverage due to a qualifying event, they can be enrolled if the employee completes the enrollment form within 30 days of the event. Benefits will be effective the month following the date the form was signed.

Terminating Employment
If the employee leaves LSUS employment or the employee/family member loses coverage, the dental plan will end at the last day of the month in which employees leaves LSUS or coverage is lost.

Cobra
The employee and/or family member will be able to continue health insurance through COBRA.
The employee will have 60 days from the date of loss of coverage to enroll in COBRA.
Coverage will be reinstated from the date of loss of coverage.
Coverage will be reinstated once COBRA enrollment forms are completed and returned along with the premium payment.
The third party administrator that administers COBRA for Dearborn is Conexius.
Conexius will send a COBRA packet directly to the employee's address that includes information regarding COBRA and the enrollment documents.

Retirees
Retirees are able to continue their dental coverage for themselves and their eligible family members through retirement at the same premium rate as an active employee.
This benefit is not automatically continued through retirement.
If the retiree wishes to continue coverage, they must notify the Human Resources Department.
Conexius is the third party administrator that will be sending retirees information regarding continuation of coverage through retirement.
Retirees don't have the option to have this deducted from their LASERS or TRSL retirement checks.
All retirees will be billed through Conexis 877-722-2667.
