2014 Annual Enrollment Information
2014 Annual Enrollment be held from October 1, 2013 - October 31, 2013 with changes being effective January 1, 2014. If you are already enrolled in vision and you wish to continue your coverage, you do not need to do anything. If you wish to enroll, make changes or cancel coverage, please see below for the Enrollment / Change form. All changes must be submitted to the Office of Human Resource Management no later than 4:30 p.m. on October 31, 2013. There are no plan changes for the 2014 year and there are no premium increases!
New Enrollment or Making Changes
Please use the attached form to enroll or make changes to your current vision plan. If you are already enrolled in the vision plan and wish to continue your coverage, you do not need to do anything. Your coverage will automatically roll over to the next plan year.

DAVIS VISION PLAN INFORMATION

Davis Vision Information Sheet (LSU First members and Non-LSU First members)
Attached is the Davis Vision information sheet. The vision plan offers some enhanced benefits for LSU First (AETNA) members. There are separate columns for the benefits for LSU First and Non-LSU First members.

To visit a list of in-network providers, please visit www.davisvision.com, to Members / Open Enrollment Plan, then enter client code of 4884 for LSU First Enrollees or 4885 for Non-LSU First Enrollees.
Any active employee of LSUS is eligible for vision insurance provided the following:
Employed at 75% of full-time effort or greater (at least 30 hours per week);
Employees may select coverage for themselves and dependents.
Children are eligible for coverage from 14 days up to age 26.
If LSUS employees have a spouse who also works for LSUS or another LSU System campus, family members may only be covered under one policy.
New Hire/Newly Eligible Employee
Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligible for benefits.
Existing employee/family member
Employees can only enroll during annual enrollment which is within the month of October. Benefits will be effective January 1st of that year. If the employee/family member lost coverage due to a qualifying event, they can be enrolled if the employee completes the enrollment form within 30 days of the event. Benefits will be effective the month following the date the form was signed.
If the employee leaves LSUS employment or the employee/family member loses coverage, the vision plan will end on the last day of the month in which employees leaves LSUS or coverage is lost.
The employee and/or family member will be able to continue vision insurance through COBRA. Coverage will be reinstated from the date of loss of coverage. Coverage will be reinstated once COBRA enrollment forms are completed and returned along with the premium payment. A COBRA packet will be sent directly to the employee's address that includes coverage information, premiums and the enrollment documents.
Retirees are able to continue their vision coverage for themselves and their eligible family members through retirement at the same premium rate as an active employee. Retirees will pay premiums directly to the provider.
