New Driver Memo


As a new employee at LSUS, you may be required to drive a state vehicle, or a rented vehicle, or your personal vehicle in course and scope of your job duties. In order to be authorized to drive for the state, you must follow our Safe Driving Program.  Please review the attached memo for more details.

Trip Travel: Request Form


Student Trip Travel Insurance provides coverage for a student while attending or participating in a university supervised and sponsored activity or function. The Student Trip Travel Insurance Policy is a minimal medical policy for eligible charges not in excess of usual and customary charges for accidents only and does not cover sickness.

Effective October 1, 2008, Student Trip Travel Insurance is 8¢ per student per day. To purchase Student Trip Travel Insurance, the Trip Travel Request Form and the List of Students must be filled out and sent to the Cashier's Cage (Accounting Services) located on the first floor of the Administration building. 

If privately owned vehicles are used for the trip, the driver of each privately owned vehicle must complete a Certificate of Privately Owned Vehicles and the faculty sponsor must approve the Certificate. All drivers on trips must be on the LSUS Authorized Driver List.

Copies of the Trip Travel Request Form (indicating payment) and the list of students should be forwarded to the Office of Human Resource Management along with completed Certificates of Privately Owned Vehicles (if private vehicles are used). These forms shall remain on file at the department level. The original may be requested if a claim is filed.

Please contact campus police immediately in the event an accident has occurred.

Trip Travel: Certificate of Privately Owned Vehicle


Student Trip Travel Insurance provides coverage for a student while attending or participating in a university supervised and sponsored activity or function. 

If privately owned vehicles are used for the trip, the driver of each privately owned vehicle must complete a Certificate of Privately Owned Vehicles and the faculty sponsor must approve the Certificate. All drivers on trips must be on the LSUS Authorized Driver List (PS 6.13.00).

Copies of the Trip Travel Request form (indicating payment) and the list of students should be forwarded to the Office of Human Resource Management along with completed Certificates of Privately Owned Vehicles (if private vehicles are used). These forms shall remain on file at the department level. The original may be requested if a claim is filed.
