Office of Group Benefits: Term Life Insurance (Prudential)

 

  • Two different plans to choose from: the Basic Plan & the Basic + Supplemental Plan
  • Flat rate of $.50/$1,000 of coverage for both plans
  • Basic Plan: $5000 of coverage
  • Basic + Supplemental Plan: Coverage is 1.5 times your salary. The maximum amount of coverage is 1.5 times salary up to $50,000
  • Can continue into retirement at same rate
  • Employee/family member needs to contact the Human Resources to file a claim

LSU Voluntary Term Life Insurance

The LSU System announced that The Hartford has been chosen as the new plan administrator for the LSU System Life Insurance Plan effective July 1, 2009. The Open Enrollment period for this Plan is during the month of May with an effective date of July 1, 2009. However, The Hartford will also offer another Open Enrollment period during the month of September with an effective date of November 1 for those employees who miss the May enrollment deadline.

Employees who are currently enrolled in the LSU System Life Insurance Plan through ING will automatically be converted to The Hartford Plan. If you are currently enrolled in the LSU Voluntary Life Plan through ING, the only reason to submit an enrollment form is to increase your and/or dependent’s coverage. Employees and/or dependents who are currently under ING’s review for medical underwriting will also be given the opportunity to elect The Hartford’s Plan during May and September on a Guaranteed Issue basis. During the months of May and September, every eligible employee will be allowed to enroll or increase their coverage up to 5 times their annual base salary up to $500,000 guaranteed issue. Employees will also have the opportunity to elect coverage on their spouses up to ½ of the employee’s amount for a maximum of $ 100,000 guaranteed issue.

Enrollment Forms, a Plan Summary sheet outlining detailed information regarding this change, and brochures for the extra added benefits with The Hartford Life Insurance Plan are listed below. This information is also available in the Office of Human Resource Management, Administration Building, AD 109.

Informational meetings conducted by The Hartford will be held during the open enrollment period in September.

Accidental Death & Dismemberment (AD&D)

 

Accidental Death & Dismemberment (AD&D)
(administered by The Hartford)
This benefit pays in the event of death or dismemberment resulting from an accident. This benefits is offered to eligible employees and eligible family members.

Eligibility
Any active employee of LSUS is elgibile for health insurance provided the following:
  • Employed at 75% of full-time effort or greater (at least 30 hours per week);
  • Appointed for a duration of at least one semester or 121 days or greater
Enrollment
  • New Hire: May enroll at any time without medical underwriting. Benefits are effective the first of the month following date of hire
  • Current employee: May enroll at any time without medical undewriting. Benefits are effective the first of the month following when enrollment form is signed
  • Newly eligible employee or family member: May enroll at any time without medical underwriting. Benefits are effective the first of the month following when enrollment form is signed

Terminating Employment
If you leave LSU employment, your AD&D policy will end at the last day of the month in which you leave LSU. You will be able to convert your AD&D policy after employment. Please complete the form below and forward it to Hartford directly to convert your policy.

Retirees: You will be able to continue your AD&D coverage for you and your eligible family members. Retirees must elect to continue coverage.
