Dental Insurance


  • provides for preventive, basic, major and orthodontic dental care
  • pays for covered expenses based on a sliding scale which is dependent on number of years enrolled in the plan
  • may see any dentist of your choice
  • two options: Enhanced Insurance Plan or Basic Insurance Plan

Eligibility
Any active employee of LSU is eligible for health insurance provided the following:

  • Employed at 75% of full-time effort or greater (at least 30 hours per week);
  • Appointed for a duration of at least one semester or 121 days or greater

Enrollment

New Hire/Newly Eligible Employee:  Employees must enroll within 30 days of hire or effective date of when employee became eligible for benefits. Benefits are effective the first of the month following employee's first full calendar month of employment or effective date of when employee became eligibile for benefits.

Existing employee/family member:  Employees can only enroll during annual enrollment which is within the month of April. Benefits will be effective July 1st of that year. If the employee/family member lost coverage due to a qualfying event, they can be enrolled if the employee completes the enrollment form within 30 days of the event. Benefits will be effective the month following the date the form was signed.

Terminating Employment
If the employee leaves LSUS employment or the employee/family member loses coverage, the dental plan will end at the last day of the month in which employees leaves LSUS or coverage is lost.

COBRA

  • The employee and/or family member will be able to continue health insurance through COBRA.
  • The employee will have 60 days from the date of loss of coverage to enroll in COBRA.
  • Coverage will be reinstated from the date of loss of coverage.
  • Coverage will be reinstated once COBRA enrollment forms are completed and returned along with the premium payment.
  • The third party administrator that administers COBRA for Ameritas is Conexius.
  • Conexius will send a cobra packet directly to the employee's address that includes information regarding COBRA and the enrollment documents.
  • The COBRA premiums will remain the same as active employees.

RETIREES

  • Retirees are able to continue their dental coverage for themselves and their eligible family members through retirement at the same premium rate as an active employee.
  • This benefit is not automatically continued through retirement.
  • If the retiree wishes to continue coverage, they must notify the Human Resources Department.
  • Conexius is the third party administrator that will be sending retirees information regarding continuation of coverage through retirement.
  • Retirees don't have the option to have this deducted from their LASERS or TRSL retirement checks.
  • All retirees will be billed through Conexius.
