2009-2010 Financial Aid Application Forms
If you are applying for financial aid for Fall 2009, Spring 2010 and/or Summer 2010 the following documents may be necessary for you to submit:
The Free Application for Federal Student Aid 2009-2010
Fill out the Free Application for Federal Student Aid 2009-2010 if you are filing for financial aid for the Fall 2009, Spring 2010, and/or Summer 2010 sessions online using your browser.
2009-2010 Student Data Form
All students applying for federal financial aid are required to complete a Student Data Form. Please print, complete and submit this document to the Financial Aid Office.
If your Student Aid Report (SAR), the result of the Free Application for Federal Student Aid, is selected for review in a process called verification you will need to submit the following documents (This list is not all inclusive. If additional documentation is required you will be notified.):
- Signed copies of 2008 Income Tax Returns used in preparing the Free Application for Federal Student Aid.
- 2009-2010 Verification Worksheet for Dependent Students
If parental income and asset information was required when you complete the application you will need to complete and submit this worksheet to the Financial Aid Office. Please print, complete and submit the document to the Financial Aid Office.
- 2009-2010 Verification Worksheet for Independent Students
If parental income and asset information was not required when you complete the application you will need to complete and submit this worksheet to the Financial Aid Office. Please print, complete and submit the document to the Financial Aid Office.
The following documents may also be used to complete the verification process:
Graduate Letter
If you are a first term graduate student, please complete and submit the Graduate Letter. The purpose of this letter is for your academic advisor to notify you if you are required to complete undergraduate course work before you can begin working on a graduate degree. Students who are required to take undergraduate courses are not graduate students until they are enrolled in graduate courses.
Summer Student Data Form If your plans are to attend LSUS during the upcoming Summer 2010 term and you are receiving financial aid for the Spring 2010 term and have not been awarded for the Summer, please complete this form. Please note that this form is not available until February 2010.
Special Circumstance Forms
Dependency Override
Complete this document if you believe you qualify for a change in your dependency status.
Income Adjustment Student
Complete this document if you income is significantly less than what was reported on your FAFSA.
Income Adjustment Parent
Complete this document if your parent’s income is significantly less than what was reported on your FAFSA.
Appeal Form
Complete this form only if you have been notified that you are not meeting the academic progress policy at LSUS.

If you are applying for financial aid for the Fall 2008, Spring 2009 and/or Summer 2009 terms the following documents may be necessary:
The Free Application for Federal Student Aid 2008-2009
Fill out the Free Application for Federal Student Aid 2008-2009 if you are filing for financial aid for the fall 2008, spring 2009, and/or summer 2009 terms online using your browser. This application is required for all students applying for federal, state and some institutional financial aid.
2008-2009 Student Data Form 
All students applying for federal financial aid are required to complete a Student Data Form. Please print, complete and submit the document to the Financial Aid Office.

Summer 2009 Enrollment Form
If you plan on attending LSUS during the upcoming summer 2009 term and you are receiving financial aid for the spring 2009 and have not been awarded for the summer, please complete this form. “Please note that this form is not available until February 2009.”

If you are a first-semester graduate student, please complete and submit the Graduate Letter. The purpose of this letter is for your academic advisor to notify you if you are required to complete undergraduate course work before you can begin working on a graduate degree. Students who are required to take undergraduate courses are not graduate students until they are enrolled in graduate courses.
Financial Aid Appeal Form 
A Financial Aid Appeal is a request that asks the Student Financial Aid and Scholarship Committee to reinstate your ability to receive federal financial aid based on your specific circumstances. In order to file a Financial Aid Appeal, you must submit a letter of appeal along with this signed form to the Financial Aid Office. Your letter of appeal should be brief (approximately one page) but thorough.
