2008-2009 Financial Aid Applications & Forms

If you are applying for financial aid for the Fall 2008, Spring 2009 and/or Summer 2009 terms the following documents may be necessary:

The Free Application for Federal Student Aid 2007-2008
Fill out the Free Application for Federal Student Aid 2008-2009 if you are filing for financial aid for the fall 2008, spring 2009, and/or summer 2009 terms online using your browser. This application is required for all students applying for federal, state and some institutional financial aid.


2008-2009 Student Data Form Requires the Free Adobe Acrobat Reader
All students applying for federal financial aid are required to complete a Student Data Form. Please print, complete and submit the document to the Financial Aid Office.

If your Student Aid Report (SAR), the result of the Free Application for Federal Student Aid (FAFSA), is selected for review in a process called verification you will need to submit the following documents (Note: this list is not all inclusive. If additional documentation is required you will be notified.):

  • Signed copies of 2007 Federal Income Tax Returns used in preparing the FAFSA
  • 2008-2009 Verification Worksheet for Dependent Students Requires the Free Adobe Acrobat Reader
    if parental income and asset information was required when you filed the FAFSA you will need to complete and submit the worksheet to the Financial Aid Office. Download and print the document using your browser and Adobe PDF.
  • 2008-2009 Verification Worksheet for Independent Students Requires the Free Adobe Acrobat Reader
    if parental income and asset information was required when you filed the FAFSA you will need to complete and submit this worksheet to the Financial Aid Office. Download and print this document using your browser and Abode PDF.
  • Worksheet ABC from the Free Application for Federal Student Aid. Requires the Free Adobe Acrobat ReaderDependent students complete both columns, independent students complete student/spouse column only. Download and print the document using your browser and Adobe PDF. (The Worksheet ABC is the last page.)

Summer Student Data FormRequires the Free Adobe Acrobat Reader
If you plan on attending LSUS during the upcoming summer 2009 term and you are receiving financial aid for the spring 2009 and have not been awarded for the summer, please complete this form. “Please note that this form is not available until February 2009.”

Graduate LetterRequires the Free Adobe Acrobat Reader
If you are a first-semester graduate student, please complete and submit the Graduate Letter. The purpose of this letter is for your academic advisor to notify you if you are required to complete undergraduate course work before you can begin working on a graduate degree. Students who are required to take undergraduate courses are not graduate students until they are enrolled in graduate courses.

 


 

2007-2008 Financial Aid Applications & Forms

If you are applying for financial aid for the Fall 2007, Spring 2008 and/or Summer 2008 the following documents may be necessary:
