(Complete packet including Immunzation forms)
Once you have completely filled out the application, print it, sign it and mail it and your fee to the address located on the application, you can also drop it off at the Admissions and Records Office.

Admission to Candidacy Form
For complete admission details, please consult the 2008-2009 LSUS catalog. Previous catalogs describe a procedure that is no longer used, and only the 2008-2009 catalog process is valid (pages 137-138).

Reference Form
Precandidates in the Specialist in School Psychology Program should submit three references. One reference must be from a professor at LSUS, one reference must be from an employer (current or past), and one reference should be from a person of the candidates choice.

Graduate Assistant Application Form
This form should be completed by those precandidates and candidates who wish to apply for one of the two graduate assistantships. The form should be returned to the director of the Specialist program.

Graduate Studies Graduate Assistant Form
This form should be completed by the precandidate or candidate after their appointment as Graduate Assistant. Please return to the program director.
