Application Information

In order to be admitted to LSUS, as a re-entry student you must submit the following items to the Admissions and Records Office:

1. If you have been out of LSUS more than one term: an Application for Undergraduate Admission or an Application for Graduate Admission . No application fee is required.
2. An immunization record
3. Official transcripts from each institution attended. You may use our Official Transcript Request Form .

Students intending to return to LSUS for summer only must complete a Summer Only Student Statement of Eligibility .
