Undergraduate Application for Admission 
Once you have completed the printed admissions application, mail it and your application fee to the LSUS Admissions and Records Office. You may also submit the application and fee in person in the Admissions and Records Office (Room 168 of the Administration Building).
*Admissions and Records Office will only accept faxed applications if they are legible.
Graduate Application for Admission

Immunization form 
Once the form has been completely filled out, sign it and mail it to the address located below on the application, you can also drop it off at the Admissions and Records Office.
Transcript Request Form 
Submit this completed form to the LSUS Admissions and Records Office to request an official transcript of your LSUS academic record.
Summer Only Student Statement of Eligibility form 
Students attending LSUS for the summer only must complete this form.
Duplicate Diploma Order Form 
To officially order a duplicate diploma, please print and complete this form.

Financial Affidavit 
International applicants must indicate the availability of sufficient funds to meet all costs while studying at the University by completing the Financial Affidavit and providing a certified bank statement. |
