Financial Aid Forms and Applications

2017-18 FINANCIAL AID DOCUMENTS
If you are applying for financial aid for the Fall 2017, Spring 2018, and/or Summer 2018 semesters, the following documents may be necessary to complete the application process:

The Free Application for Federal Student Aid 2017-18
Click the web address to complete the Free Application for Federal Student Aid (FAFSA) www.fasfa.gov to apply for federal and state grants, federal work-study, and student loans. After your FAFSA is processed, both you and LSUS will receive your Student Aid Report (SAR). LSUS will use the SAR to determine which additional documents are required.

Student Data Form
The Student Data Form is completed electronically on your myLSUS account and is only available after your SAR is received at LSUS. When you logon to your myLSUS account for the first time after your SAR is received, you will be prompted to complete this electronic document. If you do not have access to a computer, please contact the Financial Aid Office.

Summer Data Form
If you received financial aid at LSU Shreveport for fall 2017 and/or spring 2018, you may be eligible to receive financial aid for summer 2018. To determine if you are eligible for summer 2018 financial aid, please submit the Summer Data Form to the Financial Aid Office. You do not need to submit the Summer Data Form if you are new to LSUS for summer 2018 or if you're enrolled in a 100% online accelerated graduate program including MBA, MHA, NPA, MEDCI (online only), and MEDL (online only).

Master Promissory Note and Entrance Counseling for Direct Loans
All new borrowers and transfer students must complete a new Master Promissory Note (MPN) and Entrance Counseling at www.studentloans.gov . You will use your FSA ID and password to log into studentloans.gov . LSUS will receive notification within 24-48 hours after completion.
**When completing loan documents, please be sure to select the MPN for Subsidized/Unsubsidized Loans. DO NOT COMPLETE A GRAD PLUS MPN. (even if you are a graduate student)**

Verification Forms
If your SAR is selected for review in a process called "verification" you will need to submit additional documents. Additionally, there are time limits that you must meet. For students eligible for a Pell Grant, documents must be received by 8/31/2018 or 60 days after the last day of enrollment, whichever is first. For students eligible for a student loan, documents must be received 30 days prior to the last official day of class for the loan period. Verification documents are listed below. (This list is not all inclusive, if additional documents are required you will be notified):
NOTE: Please make sure that you are downloading and completing the correct verification worksheet by checking your myLSUS account or by calling the office after your FAFSA has been processed. Please also check your myLSUS account to determine if you will need to submit Federal Tax Return Transcripts.

2017-18 Asset Verification Form

2017-18 Identity Document
If you have received a verification worksheet that requires you to prove your identity AND you cannot physically come to the Financial Aid Office you may submit the Identity Document. This document must be completed, notarized and mailed to the Financial Aid Office. NOTE: This document cannot be faxed or emailed.

2017-18 Citizenship Document
If you have received a request to provide citizenship status, complete the Citizenship Document and bring an original form of government issued identification in person to the Financial Aid Office.

2017-18 Certification of True Copy Citizenship Document
If you received a request to provide citizenship status AND you cannot physically come to the Financial Aid Office you may submit the Citizenship Document and Certification of True Copy Document.



2016-17 FINANCIAL AID DOCUMENTS

Summer Data Form
If you received financial aid at LSU Shreveport for fall 2016 and/or spring 2017, you may be eligible to receive financial aid for summer 2017. FINANCIAL AID APPEAL DOCUMENT 
If you have been notified by the Financial Aid Office that you cannot receive federal financial aid because you are not making satisfactory academic progress and you have mitigating circumstances you wish to document, you may appeal to the Student Financial Aid and Scholarship Committee. Submit the Appeal Form below, along with your written letter and neccessary documentation to the Financial Aid Office. The committee meets monthly and you will be notified in writing of the results of your appeal.
