2013-14 FINANCIAL AID APPLICATION FORMS

If you are applying for financial aid for the Fall 2013, Spring 2014, and/or Summer 2014 the following documents may be necessary to complete the application process:

The Free Application for Federal Student Aid 2013-14

Click on the web address to complete thre Free Application for Federal Student Aid (FAFSA), www.fafsa.gov.  By completing this application you are applying for federal and state grants, work study, and student loans.

Student Data Form

The Student Data Form is completed electronically on your myLSUS account.  After your Student Aid Report (SAR), the result of your FAFSA, has been received at LSUS, when you logon the first time to your myLSUS account you will be prompted to complete this document.  If you do not have access to a computer, please contact the Financial Aid Office and a paper document will be mailed.

TEACH Grant Counselor's Letter

If you are applying for a TEACH Grant print this letter for an advisor in the Department of Education to complete.  Follow the instructions on the letter.

VERIFICATION

If your Student Aid Report (SAR), the result of the FAFSA, is selected for review in a process called "verification" you will need to submit additional documents.  These documents must be received before you will be awarded federal financial aid.  Additionally, there are time limits that you must meet.  For students eligible for a Pell Grant by 8/31/2014, or within 60 days after the last day of enrollment, whichever is first.  For students eligible for a student loan 30 days prior to the last official day of class.  Following is a listing of the documents required for verification (This list is not all inclusive, if additional documents are required you will be notified.):

Beginning with the 2013-14 academic year the U.S. Department of Education (DOE) expanded verification.  Please make sure that you download and complete the correct verification worksheet by checking your myLSUS account after your FAFSA has been processed by DOE or by calling the office.  If copies of income tax transcripts are required with your verification worksheet that fact will be noted on the worksheet.

If parental income and/or asset information was required when you completed the FAFSA, you will need to complete and submit this worksheet.  Please print, complete, and submit this worksheet along with the other required documents to the Financial Aid Office. 

2013-14 Standard Verification Worksheet for Independent Students

If parental income and/or asset information was NOT required when you completed the FAFSA you need to complete and submit this worksheet.  Please print, complete, and submit this worksheet along with the other required documents to the Financial Aid Office.

If parental income and/or asset information was required when you completed the FAFSA and your myLSUS account shows that SNAP verification is required, you will need to complete and submit this worksheet.  Please print, complete, and submit this document the Financial Aid Office.

2013-14 Aggregate Verification Worksheet for Dependent Students

If parental income and/or asset information was required when you completed the FAFSA and your myLSUS account shows that aggregate verification is required, you will need to complete and submit this worksheet.  Please print, complete, and submit this worksheet along with the other required documents to the Financial Aid Office.

2013-14 Aggregate Verification Worksheet for Independent Students

If parental income and/or asset information was NOT required when you completed the FAFSA and your myLSUS account shows that aggregate verification is required, you will need to complete and submit this worksheet.  Please print, complete, and submit this worksheet along with the other required documents to the Financial Aid Office.

Identity Document

If you have received a verification worksheet that requires you to prove your identity AND you cannot physically come to the Financial Aid Office you can submit this document.  Please print, complete, have the document notorize, and submit it to the Financial Aid Office.

Click on the web address to complete the Free Application for Federal Student Aid (FAFSA),  www.fafsa.gov.  By completing this application you are applying for federal and state grants, work study, and student loans.

Student Data Form
All student applying for federal financial aid are required to complete a Student Data Form.  Please print, complete, and submit this document to the Financial Aid Office.  This form is used if you are enrolling for fall 2012 and spring 2013. 

Summer Student Data Form

If your plans are to attend LSU Shreveport during the summer 2013 and you received financial aid during the fall 2012 and/or spring 2013 semester, you may be eligible for financial aid during the summer 2013. 

TEACH Grant Counselor's Letter
If you are applying for a TEACH Grant print this letter for an advisor in the Department of Education to complete.  Follow the instructions on the letter.

Graduate Letter
If you are a first-time graduate student complete and submit the Graduate Letter.  The purpose of this letter is for your academic advisor to notify you if you are required to complete undergraduate course work before beginning graduate school.  Students who are required to take undergraduate courses are not graduate students until they are enrolled in at least six hours of graduate coursework.

Signed copies of 2011 Federal Income Tax Transcript.  You will need income tax transcripts for all income tax forms that were used to prepare the FAFSA.  NOTE:  beginning with the 2012-2013 academic year signed copies of federal income tax returns are no longer considered valid documentation of income for verification.  To obtain a tax transcript from the Internal Revenue Service go online at www.irs.gov or call 800.908.9946.

The following documents may also be required to complete the verification process (You will be notified if any of these documents are required.):

Head of Household Dependent Student
This document may be required if additional information is needed about the tax filing status of your parent(s).

Head of Household Independent Student
This document may be required if additional information is need about your tax filing status.

Income and Support Statement
This document is required if you are a dependent student and your parent(s) reported zero income on your FAFSA or if you are an independent student and you and/or your spouse reported zero income.

Appeal Form
Complete this form if you have been notified that you are not meeting the academic progress policy and you have mitigating circumstances for not meeting the standards as outlined in the policy.  This form must accompany your letter of appeal.
